Ink fading no matter what

So, I currently have about 10-15 tattoos and all of them have faded. I've been to different artists and some of them are done by me. One specific tattoo annoys the heck out of me, since it has a deep meaning. It was first done by me, then I thought I screwed up so I went to an artist who has been doing this for the past 30-ish years and it still faded and looks awful, it's the worst one yet... It doesn't matter how much I stretch the skin, what ink I use, what needles, how deep the needle goes... It feels like my skin is just pushing the ink out, no matter what! It kinda looks like in the pic below, but instead of four years its about three weeks. Help? Is it possible to have a "wrong kind of skin" for a tattoo (as in, too elastic etc?)

It looks as though it has no outline and is shaded only, if so then it has been applied using grey shade techniques and grey wash which will fade lightly depending how the wash was mixed or which pre mixed shades were used.
i have a customer with very elastically skin, first time i tattooed him was with a rotary and it was horrendous and still needs a tidy up today, last time i tattooed him using a coil machine and it went fine, all clear skin can be tattooed. (for clarification purposes clear means shaven, no moles, raised scars, broken skin, psoriasis etc)

Reasons can also bad after care (not good quality after care cream or allergic to it) , sun light; if person has some kind of condition that is slowing dow healing process: bad quality ink, or too much water added to do the shading.

i was leaning toward an aftercare problem because if it's happening with so many different artists who have been in the industry for so long it's unlikely that they would ALL be bad work or ink, or anything else not related to Alex as an individual.

I could see an unknown allergy/reaction to common ingredients being something too. I would just expect that to come along with obvious signs of inflammation. But, there are lots of things included in aftercare. Cleaning habits, how well it's kept dry, how it's dried, type of ointment, too much ointment, abrasion, scratching scabs. So many things that I think would be more likely than every artist having the same problem.

What part of the body is it and was it outlined or not, are you on any medication ?

I just re-read and noticed the picture IS NOT a picture of the tattoo in question. It's a similar finished result after only a few weeks.

Alex, would you be able to post pictures of the actual tattoo? There may be something people here would be able to pick out of your actual tattoo that would help solve the problem.

yea after looking a bit more close, those are two different tattoos. The head of the fresh is more oval where the healed is rounded?? Just the markings on the skin alone are different...lol
